If your child requires the use of an EpiPen, please read and sign below: I authorize my child to carry and self-administer, as medically necessary, her prescribed EpiPen. I confirm that my child has the knowledge and the skills to safely carry, handle and self-administer the EpiPen in school. I also understand that I am responsible for my child’s actions and agree to release, indemnify, and hold harmless NDA personnel, employees, contractors or agents of the school from lawsuits, claim expense, demand or action, etc. as it relates to my child carrying his/her own EpiPen.

I understand that emergency medical services (EMS) will always be called when epinephrine is given, whether or not the student manifests any symptoms of anaphylaxis.

Refund Policy

Your summer tuition includes a $100 per session, per child non-refundable, non-transferable deposit. Once we process your payment online, this deposit is absolutely non-refundable. You may withdraw your child from any session prior to June 15, 2026 and we will refund any payments we have received less the applicable non-refundable deposit. There is no refund of any registration fees if you withdraw your child from any session after June 15, 2026 without a physician’s note. If class is cancelled, we will refund your payment in full.
